Pre-renal or pre-tubular?

چکیده

Implication for health policy/practice/research/medical education: Acute kidney injury (AKI) is one of the most common clinical encounters during nephrology inpatient rounds. It is traditionally classified as pre-renal, intrarenal and postrenal categories. However, these terms are often confusing and not very helpful from diagnostic point of view. The frequently used simple and derived urinary indices also give conflicting results among these subtypes. It is suggested to replace these terms with pre-tubular, tubular and posttubular AKI. Evaluation of Urinary GGT in Elite Male Karate Athletes Pre & Post Exercise

Background and Aims: Post exercise proteinuria and increased urinary Gamma-Glutamyl transferase (GGT) levels can be indicative of exercise-induced renal damage. The aim of this investigation was to study the effect of one session of intensive training on renal tubular injury markers and compare their values to those 6 hours after training, for evaluating tubular damage after intensive ...

Vitamin E protection against gentamicin-induced nephrotoxicity in rats: a biochemical and histopathologic study

The specificity of gentamicin for vitamin E deficiency-associated oxidative stresses in the renal proximal convoluted tubules is apparently related to its ability to increasingly facilitate generation of radical species in mitochondria. To determine the ways in which vitamin E manage the currently processes, we conducted a prospective study aimed to investigate the tubular preserving effect of ...

Urinalysis and pre-renal acute kidney injury: time to move on

Urinary indices are classically believed to allow differentiation of transient (or pre-renal) acute kidney injury (AKI) from persistent (or acute tubular necrosis) AKI. However, the data validating urinalysis in critically ill patients are weak. In the previous issue of Critical Care, Pons and colleagues demonstrate in a multicenter observational study that sodium and urea excretion fractions a...

Correlation between endothelin expression in early post-transplant biopsy specimens and long-term allograft function in living-related renal transplantation.

OBJECTIVE We investigated whether degree of immunohistochemically evident endothelin (ET) expression in early post-transplant biopsy specimens could predict long-term allograft function in living-related renal transplantation. METHODS Allograft biopsy specimens obtained from 40 patients with living-related transplants were studied. Cases with episodes of acute rejection or calcineurin inhibit...

Time course and functional correlates of post-transplant aluminium elimination.

Urinary excretion of aluminium after a successful transplant can reverse pre-transplant aluminium intoxication. We have evaluated the time course of urinary aluminium excretion and its correlation with several parameters of renal function and mineral metabolism in 49 patients (33 men and 16 women) with a wide range of pre-transplant serum aluminium concentrations, performing sequential determin...
